ABRA PROVINCIAL HOSPITAL OFFICIAL STATEMENT ON CHOKOLEIT'S DEATH

READ: Official Medical Statement of the Abra Provincial Hospital (APH) for the case of Comedian Chocoleit

Patient is a certain Jonathan Aguilar Garcia, 48 years old, male, complained of difficulty of breathing while in the venue of a show. Our medics from Health Emergency Management Services (HEMS) noticed the patient to be gasping for breath while performing. Hence, medics were immediately sent at the backstage expecting the worst.

Comedian Chocoleit was noted to have difficulty breathing. He was advised but refused to be brought to the hospital claiming that he was ok.

However, few minutes later, he collapsed and was eventually rushed to the Abra Provincial Hospital, where a team of 4 doctors was waiting for him. En route to the hospital, he was given oxygen and put on high back rest. Vital signs were as follows; BP 140/100 CR 110 RR 50 O2 sat 40%. He was noted to have cold clammy perspiration, cyanotic and had involuntary eye movements (seizure?).

After approximately 5 minutes, he was received by doctors TerredaƱo, Gonzales, Alameda and Seares, in severe cardio-respiratory distress. Initial BP was 200/100 CR 130 RR 50 O2 sat 36% RBS 159mg%. He was intubated upon arrival at the ER and hooked to manual ambubagging. ECG showed abnormal tracings (AV dissociation). Voluminous frotty secretion was noted to be coming out of his endotracheal tube and from the oral cavitiy necessitating frequent sactioning. Tropinin I was elevated at 0.346 ng/dl.

Soon he went into cardiac arrest and chest compression was initiated. Resuscitation went on for 44 minutes.

However despite our efforts to revive him, he eventually succumbed and pronounced dead at 11:38 PM. Immediate cause (of death was) acute pulmonary edema; Antecedent cause: cardiogenic shock; underlying cause: Acute Myocardial infarction; under contributory factors: Chronic Obstructive Pulmonary Disease in acute exacerbation.
